To claim your ARKi under NDIS funding:
For Self-Managed & Plan-Managed Funds
You will need to submit a Health Professional assessment for inclusion of ARKi in your plan along with an invoice from us to NDIS or your Plan Manager. Pls fill out the attached form below and we will issue you an invoice for submission.
For Agency Managed (NDIA) Funds
We are happy to issue you a quote and submit a payment request thru the MyPlace portal using NDIS funding.
